The East Greenbush Animal Hospital staff wants to make your visit go as smoothly as possible for you and your pet. If you are a new client or bringing in a new pet, a registration form will need to be completed.  Also, please bring a copy of your pets medical history including vaccinations and surgeries.

New Client/Patient

We are proud to say that we are a “flea free facility”. In an effort to keep this status we require all animals that stay in our facility to be current on a veterinarian approved flea prevention. If you have any questions regarding this policy, please discuss it with one of our staff members prior to your pets scheduled stay.

  • I understand that I am responsible for my dog at all times. I will keep my dog “on-leash” at all times in class, except in the event the instructor requests my dog to be off-leash, in the course of training. I agree that the instructor is not being held responsible in the event of an accidental injury. I confirm that my dog is up to date on all his/her vaccinations. I understand the above information and have honestly filled out this form.
